Immunizations and Expemptions
Oklahoma law requires parents of all children attending school or childcare facilities to provide acceptable evidence of adequate immunization or an exemption. Proof is required before the child can enter or attend school.
The State Board of Health prescribes the required immunizations and their administration frequency, conforming to recognized medical practices. The Oklahoma State Department of Health supervises and enforces these immunization requirements.
Immunization Requirements for School Enrollment
Parents or guardians must provide one of the following:

- Current, up-to-date immunization records
- An approved exemption form

Exemptions
The Oklahoma State Department of Health has launched a new online portal to streamline immunization exemption requests. Parents or guardians can now submit exemption applications online for children in schools, childcare facilities, and Head Start programs.
Exemption Process

- Download the State Health Department Immunization quick sheet and scan the QR code or visit https://imm-public.osdh.immytech.com to request an exemption for your child.
- Create an account on the portal.
- Submit the exemption request online.
- Parents/Guardians are responsible for providing the approved exemption letter to the school.
Important Notes:

- Exemptions must be renewed at the end of the 6th-grade year for students entering 7th grade.
- The school will no longer submit exemption requests; parents/guardians will handle the process directly.
